These areas have some high levels of deprivation and social housing so they ae going to create a downward spiral where the poorest are the elast well cared for.

The flaw with that argument is that it was the same scenario before Obamacare was introduced.

Look at dialysis for example. The US spends the most in the world on this (mainly going to private companies) but has one of the worst mortality rates for a developed nation.

When I was resident in Belgium, we had a basic state-funded medical service which you could top up with optional private insurers if you didn't want pre-used bandages, etc.

The system worked well and wasn't expensive.

One difference in the Belgian system is that you don't really get GPs as such. Doctors tend to specialise and advertise their speciality. So, if you have asthma, you go to an asthma specialist: if you have back problems you go to a back specialist. I suffered from chronic indigestion at one time:  the doctor I found in the Yellow Pages decided I needed a slow-release implant, whipped out a scalpel, did the job there and then in his home surgery and stitched it up afterwards.

No waiting weeks for a referral.

GPs existed before the NHS and fought its construction. As far as I'm aware they're separate businesses that contract themselves out to the NHS.

Yes, but what is wrong with seeing a 'Specialist' rather than a 'Jack of All Trades'? We also have 'Hausärzte' General & Internal Practices  - Pratices that deal with the day to day stuff..  They will always pass you on to specialists if necessary. It is possible to change doctors, specialist or not, every quarter.

Practices are certainly not as grim as the British ones tend to be. My General Practioner is available for walk in cases every morning and  appointments in the afternoons.

e.g. Would you know how to self diagnose a brief episode of "dizzyness" and refer yourself to the correct specialist?

Would you refer yourself to  

1/ a cardiologist

2/ a neurologist

3/ ear, nose and throat specialist.

You have a nonspecific back pain/ ache. Would you refer yourself to

1/ orthopaedic specialist

2/ neurologist

3/ nephrologist

4/ physiotherapist

Health services which do not have a system of GP triage and referals can also have patients trying to work out who would be an appropriate specialist to see, then ringing around themselves until they find a specialist with space on their list. If they have misdiagnosed themselves and gone to the wrong specialist they start again.

I have chronic back pain. My GP has referred me for physiotherapy - no improvement. X-rays - nothing wrong. Then for MRI scan - again nothing wrong.

So I asked him "why do I have back pain when the X-ray and the MRI show no problem ? " His answer - " I don't know. Next please "

At least I managed to get myself a Blue Badge - no help from the GP.

The Tories are spending billions more on the NHS with further billions pledged in future years. Don't panic and don't trust the Left wing scare mongers at the failing Guardian. 

The NHS Chief Simon Stevens agrees with you on the billions but not the outcome and cynical propaganda. See what he told the Commons Public Accounts Committee earlier this year ....
