I arrived at the scene this evening (Sunday) at 16:16 just as an older lady had hurt her leg badly when the manhole cover she walked over on pavement at SW corner of Cavendish and Green Lanes had collapsed about 25cm. I've attached a photo of what happened.

I called the council after-hours number on 02083483148 and spoke with 'Julie' who took down the details, gave me reference number T46467K3H6 and said someone would come along to assess and make it safe.
I waited for a half an hour as a red, human traffic cone, but no-one showed yet. With a little help from the people at Dostlar's, I left it as follows:
It looks like the metal lip that held this cover up was totally gone -- maybe we should get the council to check the rest of them in the area, especially high traffic areas.

Well done for reporting this Michael and staying around to see that it was made safe. I'l be interested to learn how quickly it gets fixed.

Council officers tell me that it should be fixed this afternoon. If this isn't doine by tomorrow morning, I'd be grateful if someone could let me know.
David any chance can you also ask them to fix the one outside 16 Wightman Rd. It has been two weeks since I notified the council, the hole is a lot smaller but equally dangerous.
I checked this afternoon at about 1pm and no-one had even "made it safe" with pylons or barriers yet -- it was still the makeshift warning boxes I put over it last night.
If you report it using the "report a Problem' page on their website and mention it is health and safety risk they will fix it. I used the health and safety issue before and they acted pronto. All because if they don't there is an audit trail left which would be evidence in the light of a legal claim.
