I'm having all the works you describe done in my loft starting this week. I've hired http://www.theloftboys.co.uk/
They quoted around £3k all in, though in my case this also include the supply and installation of a window. They specialise in loft boarding and have thus far been very professional and courteous.
No worries. They say they'll be finished in 4 days so will report back here.
Just to follow up the work is almost complete - the ladder (which they install last of all so as to avoid damaging it while boarding) arrived broken, so they had to order another one. I'll upload some photos once they install that early this week and I can get up there easily.
Aside from that (which wasn't their fault), the rest of the work has been done very well and very quickly. The workmen were pleasant and were happy to go the extra mile to fulfill my requests, such as boarding right into a triangular alcove. They tidied up carefully at the end of each day.
We had the same done 10 years ago Michael for a bit less than Nick quotes here so it seems a fair price. Also, make sure they do 2 things:
1. Lift the beams that the boards will sit on above the rafters. The rood rafters are not designed to take the weight of a floor and they will likely damage your ceiling if the beans are not hung properly.
2. Get them to make sure the level of insulation is up to the minimum standard if it is not already. While the they are up there this will be a good time to do that piece of work and you will benefit from it..
Oh, and 3rd. get them to put some proper lighting up there (a couple of strip lights, so you can see while up there). Oh, lastly... make sure they use some solid insulation board to insulate the loft hatch and use the rubber draught proofing around the edges too- you would be amazed by how much heat loss there is through a loft hatch!

If you need a large loft hatch I've got one going spare. It has a ladder attached, but that's pretty knackered and will need replacing. The hatch itself is fine though. I don't have the measurements handy, but it's a pretty big one.
