 I know you have stated that you do not know her from an employment environment, but please could you answer the following questions?

 If you know the applicant from an employment environment, please answer the following questions:

             a) What is/was the applicant’s position?

             b) What are/were his/her responsibilities?

             c) How does he/she relate to management and colleagues? 

             d) Would you re-employ the applicant?  YES/NO

I'd go with Souvraine's suggestion, John!

I've replied as follows ;

a) Don't know

b) Don't know

c) Don't know

d) See a)

I assume you do know the person who's applying for the job - I would tell them that the employer has sent an inappropriate letter, so that they can offer a different referee.  The first suggestion may have been ironic, but it would really not be helpful to do that!

How about saying - I've never employed this person but I wouldn't hesitate to do so and say why
