Try the newish cafe Music & Beans (the one that sells guitars & ukuleles) I believe they do lessons there.

The tutor is called George, and the written application is made on Form B.

Try the Ukulele Wednesdays website. There's a songbook you can download free of charge and there are 3 pubs around London where people can just turn up for a 'Ukulele Jam' also free of charge (I personally go to The Duke in Wood St Walthamstow most Wednesday evenings). Plus they organise lessons too.
